The right information, with the right team.

We help connect discharge teams, facility nurses, treating practices, home health and families. Current wound orders, referral records and the next responsible contact travel with the plan. The treating clinician directs assessment and treatment.

Getting to care.

We help arrange transportation by coordinating the visit time, access needs, pickup and return details with the patient, care team and transportation provider. The provider or benefit plan confirms eligibility, booking and any costs. Plan transportation and access.

Supplies and ordered services.

We help clarify who handles wound dressings, equipment, supply orders and formulary questions. When a clinician orders imaging or another service, coordination includes the receiving provider, appointment and return of results to the treating team. Product selection and clinical orders stay with the clinician.

Follow-through on documentation.

We help practices organize records, encounter documentation and superbill transmission to the responsible billing team. Questions about coding, claim submission and CMS requirements go to the appropriate billing staff and treating practice. Documentation support does not determine coverage or payment.

A clear handoff after discharge.

Before the patient changes care settings, confirm who is receiving the plan, who will carry out each part and how questions will reach the treating clinician. Patients and loved ones should know the next step and whom to contact. Follow-up through care transitions.
