Care in the right setting.

We work with clinical partners to coordinate wound care at home and across care settings, including nursing communities and outpatient practices. The treating clinician determines the assessment, treatment and setting appropriate for you.

Help with the practical details.

  • Follow-up: connect appointments and next steps with the people involved in your care.
  • Records and supplies: help communicate orders and requests to the responsible practice or supplier.
  • Transportation: help arrange transportation with your care team, health plan or a transportation provider.

Your family and care team.

You decide whom to include in the conversation. With your permission, we coordinate with family, caregivers, primary care, specialists and the nurses already involved. Information for families and loved ones.
