Patient and family callback requests collect a name, phone number and permission to contact you. Professional inquiry forms also ask for an organization and role. The forms include a hidden field to help discourage automated submissions.
Use the forms for your contact details only. Do not include diagnoses, wound descriptions, patient records, photos, insurance identification numbers or Social Security numbers. Professional inquiries should not identify the patient.
How inquiry information is used.
Gateway uses submitted contact details to review your inquiry and respond. Public form submissions are delivered to contact@gatewaywoundcare.com. Gateway does not sell public website form submissions.
Clinical information.
Contact Gateway for instructions before sharing clinical records or other sensitive information. This notice covers the public website; the treating practice provides its own care-related privacy information.
How care is arranged.
An inquiry begins a conversation. Clinical care, appointments and any coverage requirements are confirmed with the responsible practice. Submitting a website form does not by itself establish a treating relationship.
Technical information.
Hosting and delivery services may process IP addresses, browser information and request logs to operate and protect the website. Do not use website forms or ordinary email to send medical records.
