Tell us what is difficult to manage.
Contact us about a parent, partner, relative or friend. Appointments, wound-care instructions, supplies or transportation may be the immediate concern. We can help identify the next step and the people who need to be involved.
Keep the plan and contacts together.
- Keep the current care instructions and care-team phone numbers in one place.
- Ask who handles appointments, supply orders and questions between visits.
- Tell the team if transportation, time or support at home makes follow-up difficult.
- Bring questions to visits and ask for explanations in everyday language.
Include the people your loved one chooses.
The patient’s preferences guide whom we involve. We coordinate with the treating team and designated family members so practical arrangements reflect the care plan. Clinical questions and changes in condition belong with the treating clinician.
