- Begin with your organization, direct contact, patient city or ZIP, care setting and anticipated discharge timing.
- Identify the receiving facility or home contact and the clinicians already involved.
- Ask Gateway how to provide discharge notes, current wound orders and other requested records securely.
- Confirm referral acceptance, visit availability and the next responsible contact before finalizing follow-up arrangements.
- Confirm transportation, access, supply arrangements and who will follow up on any clinician-ordered imaging or other pending service.
- Keep patient names, diagnoses, photographs and clinical records out of the public callback form.
