- Share the facility type, location and approximate number of residents needing wound follow-up.
- Identify the nursing contact, attending physicians and existing wound-care arrangements.
- Discuss visit frequency, documentation and communication based on resident needs.
- Agree how changes between visits will reach the responsible clinician.
- Include discharge planning so the next care team receives the current wound plan.
