The treating clinician assesses the patient, decides the care plan and issues clinical orders. Gateway helps coordinate inquiries, records, appointments and communication with the patient, family, facility and other providers.
Mobile care and care across settings.
Care coordination includes home visits, facility wound rounds and outpatient care through partner practices. The patient’s clinical needs guide the setting. Scheduling, records and follow-up should remain connected when the setting changes.
When travel is needed, we help arrange transportation with the care team, health plan or a transportation provider.
Orders, imaging and supplies.
We help connect clinician-ordered imaging with scheduling and the return of reports to the treating team. Supply and durable medical equipment requests are coordinated with the responsible practice and supplier, including order documentation and facility formulary requirements. Clinical selection and interpretation remain with the treating professionals.
Documentation and billing workflows.
Practices can discuss administrative support through Gateway Care Partners, including encounter-document and superbill transfer, claim preparation, authorized submission and follow-up. CMS-1500 and electronic 837P workflows depend on the practice’s billing authorization and the payer’s requirements; a completed handoff is not a guarantee of payment.
Continuity beyond a single visit.
Primary care, chronic care management, advanced primary care management and remote patient monitoring each have distinct clinical and payer requirements. Where these programs are part of a partner practice’s care, coordination helps connect the patient’s wound needs with the broader plan.
