- Clarify who provides clinical assessment, routine nursing visits, dressing care and supplies.
- Keep current orders and relevant wound updates available to the clinicians involved.
- Establish a direct contact for new concerns or questions about the plan.
- Review responsibilities again after hospitalization, a treatment change or a move between care settings.
