Work with the resident’s care team.
Gateway helps coordinate inquiries, records and visit arrangements with the resident, designated family members, community staff and treating clinicians. The treating practice determines the appropriate clinical care and setting.
Clarify support between visits.
Identify who handles appointments, communicates with the clinician and helps obtain ordered supplies. The resident’s existing support, including any home-health services, guides these arrangements. Community staff are not assigned clinical tasks beyond their role.
Address travel and transitions.
If a visit requires travel, we help coordinate transportation arrangements with the resident, family, care team, health plan or a transportation provider. Changes in care setting should include current instructions and a clear follow-up contact.
