Who Can Refer?

Anyone can initiate a referral for wound care. We accept referrals from:

Healthcare providers: Physicians, nurse practitioners, physician assistants, discharge planners, social workers, and care coordinators
Facility staff: Directors of Nursing, charge nurses, and administrators at skilled nursing facilities, assisted living communities, and group homes
Home health agencies: Care coordinators and home health nurses
Patients and families: Patients can refer themselves, or a family member or caregiver can contact us on their behalf

What Happens After You Refer

Our care coordinator receives your referral and confirms the details.
We verify the patient's insurance and obtain any necessary authorizations.
We contact the patient (or facility) to confirm location and schedule the initial assessment.
A board-certified wound care specialist visits the patient, usually within 24 to 48 hours.
You receive a detailed report after the initial assessment and after each subsequent visit.
