Who Can Refer?
Anyone can initiate a referral for wound care. We accept referrals from:
- Healthcare providers: Physicians, nurse practitioners, physician assistants, discharge planners, social workers, and care coordinators
- Facility staff: Directors of Nursing, charge nurses, and administrators at skilled nursing facilities, assisted living communities, and group homes
- Home health agencies: Care coordinators and home health nurses
- Patients and families: Patients can refer themselves, or a family member or caregiver can contact us on their behalf

What Happens After You Refer
- Our care coordinator receives your referral and confirms the details.
- We verify the patient's insurance and obtain any necessary authorizations.
- We contact the patient (or facility) to confirm location and schedule the initial assessment.
- A board-certified wound care specialist visits the patient, usually within 24 to 48 hours.
- You receive a detailed report after the initial assessment and after each subsequent visit.
